Connect and Evolve was established from the need to offer Support Coordination and Psychosocial Recovery Coaching through a personalized approach.

 

In a liquid society where everything is constantly changing, the NDIS is also rapidly evolving.  Unfortunately, the community services sector is becoming heavily influenced by a business approach, and this makes it is easy to forget that the people we work with are the pivotal point of it all.

 

Connect and Evolve strikes to provide tailored support where waitlists, generic procedures and fine print are no longer present.

 

Our goal is to offer Support Coordination and Psychosocial Recovery Coaching on a personal level, where participants and professionals can work together towards discovering and achieving goals through a holistic lens.
